

GUIDELINES ON HOW TO APPLY FOR AN
E-PASSPORT
FIRST TIME APPLICANT
1. Dial the Telephone Passport Express Hotline at (02) 567-1111 to learn the process and get your reference number.
2. Prepare the required documents. Click here for application form.
3. Submit fully the accomplished Application forms (please leave no blanks) with required documents either through:
• Duly accomplished application form with Telephone Passport Express logo.
• Original Birth Certificate on National Statistics Office (NSO) security paper.
• Any two (2) of the following valid IDs:
ADDITIONAL REQUIREMENTS
a. For Married women
• Marriage Certificate on NSO security paper
• If married to a foreigner, Certificate of Filipino Overseas (CFO)
b. For Minor applicant (below 18 years old)
• Original passport of travelling companion
• If travelling with father, Marriage Certificate on NSO security paper
• If travelling with guardian:
c. For Seaman
• Seaman’s book
d. For Muslim applicants
• For those whose births were registered, Please refer to requirements for first time passport applicants.
For those whose births were not registered:
• Late registered Birth Certificate (BC)on NSO security paper
• Original and photocopy of Voter’s Affidavit or other supporting documents indicating date
and place of birth and citizenship, identity and tribal affiliation.
• Certificate from the Office on Muslim Affairs (OMA)
For converts:
• Submit annotated Birth Certificate (BC) on NSO security paper
• Court Order granting change of name
• Shari’ah Court Order
• OMA Certificate of Conversion

documents or personal appearance from the applicant.

REQUIREMENTS FOR E-PASSPORT RENEWAL APPLICANT
* PERSONAL APPEARANCE IS REQUIRED *
• Duly accomplished application form with Telephone Passport Express logo
• Old passport for cancellation
• Photocopy of 1st, 2nd, 3rd and last pages of the old passport including the latest arrival/departure stamp, if used
• Any one (1) of the following valid IDs:

a. For Brown Passport (Issued on or before 1995)
• Any documents with complete and full middle name, such as any of the following:

• Marriage Certificate on NSO security paper
• If married to a foreigner, Certificate of Filipino Overseas (CFO)
c. For Minor applicant (below 18 years old)
• Original passport of travelling companion
• Photocopy of Birth Certificate on NSO security paper

NOTE: There will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.
