Passport application HOW TO APPLY FOR A PHILIPPINE E-PASSPORT

GUIDELINES ON HOW TO APPLY FOR AN E-PASSPORT FOR FIRST TIME APPLICANTS

Step 1: Dial the Telephone Passport Express Hotline at (02) 567-1111 to know more about the process and to get your reference number
Step 2: Prepare the required documents. Click here for application form
Step 3: Submit the fully accomplished Application form/s (please leave no blanks) with the required documents either through:

  • a. Pick up
  • b. The Telephone Passport Express Center
  • c. The nearest LBC Branch
    Step 4: Fill up delivery form and pay the necessary fees
Step 5: Your submitted documents will be subjected for review and additional requirements will be coordinated to you if needed
Step 6: Wait for our text or call regarding the exact schedule of your personal appearance
Step 7: Proceed to the DFA on the assigned date and time
Step 8: Your new passport will be delivered to you within 24 hours upon release via LBC Door-to-Door Delivery Service

REQUIREMENTS FOR FIRST TIME E-PASSPORT APPLICANT
* PERSONAL APPEARANCE IS REQUIRED *

• Duly accomplished application form with Telephone Passport Express logo
• Original Birth Certificate from the National Statistics Office (NSO)
• Any two (2) of the following valid IDs:

  • Social Security ID/Government Service Insurance System ID
  • Computerized Voter’s ID
  • Professional Regulatory Commission ID
  • LTO Driver’s License with Official Receipt (OR)
  • School ID with original registration card/transcript of records
  • Or any old documents with date and place of birth

Note: TO ENSURE APPLICATION APPROVAL, ADDITIONAL REQUIREMENTS MAY BE REQUESTED UPON REVIEW OF SUBMITTED DOCUMENTS

Warning: Submission of false information and falsified documents are punishable under the Philippine Law. It is up to the discretion of the DFA to accept, reject or request for additional supporting documents or personal appearance from the applicant.

• Any one (1) of the following valid IDs:

  • Professional Regulatory Commission ID
  • LTO Driver’s License with Official Receipt (OR)
  • School ID with original registration card/transcript of records
  • Or any old documents with date and place of birth

  • a. Pick up
  • b. The Telephone Passport Express Center
  • c. The nearest LBC Branch
    Step 4: Fill up delivery form and pay the necessary fees
Step 5: Your submitted documents will be subjected for review and additional requirements will be coordinated to you if needed
Step 6: Wait for our text or call regarding the exact schedule of your personal appearance and the details regarding the release of your passport record and approval for processing
Step 7: Proceed to the DFA on the assigned date and time

REQUIREMENTS FOR THE REPLACEMENT OF A LOST E-PASSPORT
* PERSONAL APPEARANCE IS REQUIRED *

  • School ID with original registration card/transcript of record
